All the Medical records presented for the case are reviewed in detail. Only the case-significant information is summarized in a chronological order to know the events surrounding the case. This is a verbatim (As-is) summarization of the records.

The medical records are reviewed, and the significant case details are presented in a succinct narrative format.

The records for a mass tort case are reviewed and summarized in different formats based on the case/client requirements and the stage of the case progression. We work on the different templates provided by our client, or we create new templates on client's request. We work on Bellwether cases and case consolidation as well.

This Initial Case screening is requested during the Pre-suit investigation stage. This helps the law firms to determine if it’s a valid claim to proceed. Hence, it saves a lot of time and effort for the law firm and reduces further costs. MD provides a succinct narrative summary of the case and describes the merit of the case based on the medical records.

The Evidence-Based Case Merit Review The MD provides a detailed medical opinion on the case with evidence from the medical records and substantiates the final opinion with medical literature. It includes a brief description of the critical events, and the medical opinion explains the duty of care required, who had the duty of care, any deviation in the duty of care, the damages, and whether the damages were caused by the deviations or not. All these are substantiated with evidence from the medical records and literature support.

MD can help with extensive medical research to support the case progression.

Based on the case type and events, a set of case-specific questions are drafted to be used during deposition.

After careful evaluation of the case, we provide projections on future treatment, support care required, lifestyle modification, employment capability, and future expenses. This helps to plan the financial requirements during the claim and ensures the patients a better future.

We identify the critical missing records required for the case during the initial and detailed review of the records. This helps the law firms to be well-equipped during the case progression.

A summary of the depositions is prepared. This highlights the key points that are supportive during trial preparation or the settlement of the case.

A demand letter is drafted based on the case review and details available, which helps in pre-litigation.

The Billing Summary is well structured, identifying the bills as related and unrelated to the case. The case-related bills will be summarized accurately, including the date, the treatment provided, the amount, and the provider details, in an Excel spreadsheet. This is usually backed up with the medical records provided for summarization.

We know that the number of records collected for any litigation process is huge and voluminous. We rename, sort, and organize these records that help law firms solve the mystery and make their life better through the litigation process.

The PFS sheet provided by the client will be filled as per instructions and the details available in the medical records.

A special report can be prepared based on the case requirement, which will highlight the key points in a diagrammatic or data form. For e.g., the pain suffered, the medications taken, flow of events, the procedures performed, pre- and post-MVA, radiology reports, pre- and post-life style changes, and many more.

Each medical record will be bookmarked with the title of the page and reorganized in a chronological order for ease of reference.

The page reference provided in the medical summary will be linked to the corresponding medical record for easy reference.

The records will be Bate-stamped based on the provider and will be indexed as per requirement to identify each record in just a few seconds. This helps easy handling of the voluminous records.
